Medical questionnaire auto fill out patient details

At the moment I either leave the medical form where it is or manually transfer details like, name, phone number, email, address, GP and medical information into the patient details.

It would be great if the medical form could automatically transfer these details over or if we could customise it to fit our own medical forms and have it automatically fill out when a patient fill out their medical form.
