Link Appointment Type to Notes Template

This feature would link appointment types to a treatment note template. For example, if an Initial appointment is booked and the appointment is opened in the diary, we could add a link to Create new treatment note that would automatically open a blank note template called Initial appointment template (for example).

This could be titled “Default note template” on the appointment types dialog window.
